The following will confirm the agreement (“Agreement”) between __________________ (“Company”), on the one hand, and __________________ (“Consultant”), on the other hand, whereby Company hereby engages Consultant to provide non-exclusive consulting services to Company, on the following terms and conditions:
1. CONDITIONS PRECEDENT. Company’s obligations under this Agreement are conditioned upon the following:
a. Company’s receipt of fully-executed originals of this Agreement; and
b. Consultant providing Company with all documents which may be required by any government agency or otherwise with respect to Consultant’s services hereunder, including, without limitation, and an INS Form I-9 (Employment Eligibility Verification Form) and/or an IRS FormW-9, completed to Company’s satisfaction.
