Appearance Release

An Appearance Release is used for anyone that you will film for your production such as someone you are interviewing or otherwise appearing in your production.  The Appearance Release is sometimes called a Depiction Release.  You will not use this form if you are hiring an actor to appear in your production.  In that case, you would use an actor form.

This is often used for subjects of a documentary, when you want to interview someone or film someone performing other than acting for you.
